GENERAL APPLICATION - FORM 3
Please note the Form 3 is not to be used for Adoption Applications.
The orders which can be sought using the Form 3 are usually ancillary to the Adoption Application and may be one or more of those listed below:

A determination that an order of adoption is preferable to any other order being made under the Family Law Act.

That the consent of the birth father of the child to be adopted be dispensed with pursuant to Section 24 of the Adoption Act 1994.

The requirement to serve the birth father of the child to be adopted is dispensed with pursuant to Section 25(2) of the Adoption Act 1994.

That the requirement for an Adoption plan be dispensed with pursuant to Section 73 of the Adoption Act 1994.

That service of a copy of the application and affidavit on the birth father is dispensed with.
